Patient Information

My  First Visit - Your first visit will involve a comprehensive periodontal examination to assess the health of your teeth and gums.  If you have been referred for a specific problem or procedure this will be addressed rather than a full mouth assessment. Time permitting, treatment may commence at the same appointment. Following this a diagnosis will be given and possible treatment options discussed with you. On the day of your visit please:-

  1. arrive five minutes prior to your scheduled time
  2. bring any referrals/letters and x-rays from your dentist
  3. bring a list of your current medication.

Please complete the New Patient Medical History form by clicking on the link below.

Note:  All patients under 18 years of age must be accompanied by a parent/guardian.

Scheduling

We will schedule your appointment for treatment as soon as possible.   There may be times when you will have to wait for a short period prior to commencing your treatment.  If your case is urgent we will attempt to accommodate you as soon as possible.

Financial

Once your treatment plan has been finalised we can then provide you with a quote including item numbers that you may present to your health fund to ascertain if you are eligible for a refund.   Rebates from health funds vary widely depending on your level of cover and these are beyond our control.   Payment for your treatment is required on the day of your procedure.   We do not provide Hi Caps at any of our surgeries and you will need to claim your refund directly from the health fund yourself.
